We provide safe and luxurious homes and apart-ments which are well furnished and personalized to suit each person. Our homes are also structur-ally modified to accommodate persons with physical disabilities.

Our Programs & Services

All the men and women who reside in our community based homes and apartments receive personalized services designed to meet their specific needs and desire for enhancing their domestic and community integration

RESIDENTIAL REHABILITATION

Metro Homes, Inc. over the course of several years has continued to offer effective Residential Habilitation services in a variety of home-like settings. These services provide opportunities for choice of living arrangements, personal care, nursing, community engagement, and transportation to the people living in the home. Services include consulting and therapeutic services as identified in the services plan consisting of Psychiatric, Psychological, Social, Occupational and Physical Therapy, Speech, Hearing and Language Therapy, Nutrition Services, and Skilled Nursing. These services are provided directly by the consulting therapist and or trained staff Direct Support Professional (DSP). Many of the activities provided in the home support training and development of independent living, personal care, social interactions, dining skills, household care and choice and decision-making skills which aid in increasing independence, self-advocacy, and community inclusion.

Nursing services provided are a part of the persons overall comprehensive plan of care (HCMP) in conjunction with the persons Primary Care Physician and support medical and preventative care activities related to serious or persistent health issues that treat and manage a condition. These services include health assessments and treatment, health related trainings, and education for persons receiving services and (DSP). Nursing services also includes ensuring other therapies are appropriately identified and implemented as needed. Metro Homes, Inc. Qualified Intellectual Disabilities Professionals and other management staff support and ensure all homes and vehicles are well maintained, staff are trained to implement the persons plans and services and supports provided focus on the needs of each person support. The QIDP’s provide oversite in all areas of each person’s care plan, skill development activities and provides ensures reports (quarterly) are completed as required.

SUPPORTED LIVING SERVICES

Metro Homes, Inc. provides Waiver Supported Living Service that covers habilitation, personal care, nursing, other residential supports and supported living services with transportation that allows people to gain access to other community services and activities. Metro Homes Waiver supports a person to have full access to the greater community including opportunities to seek employment and work in competitive integrated settings, engage in community life, control personal resources, and receive services in the community, to the same degree of access as people not receiving Medicaid HCBS. The programs are designed to optimize a person’s ability to make life choices, including choices related to daily activities, the physical environment, and with whom to interact. Metro Homes, Inc supports people choice of living arrangements/locations including settings that are not disability specific and ensures people’s rights to privacy, dignity, respect, and freedom from coercion and restraint are protected. The Waiver program also supports a person’s choice regarding services and supports and who provides them.
