General Summary:

Responsible for providing assistance to people we support physically, socially and psychologically and completing required trainings for compliance with State/Federal regulations or standards of service and general quality of service and safety of the people we support in the programs as per the Policy and Procedure of the Agency and the governing agencies. These tasks are complex and required negotiation and communication skills, organizational abilities, advocacy skills and overall business sense. Employees are also required to accompany our persons on vacation annually. 

Key Functions:

  • Provides assistance to the people we support in order to promote their physical, social and psychological well-being.
  • Interacts frequently and positively with the people we support and staff to promote the rights of the people we support to achieve an enhanced quality of life.
  • Completes all documentations as required to ensure quality services.
  • Follow agency procedure to promote optimum health care and behavioral support as well as maintain the well-being of the people we support.

Qualifications:

  • Experience in providing services and support to people we support with developmental disabilities preferred.
  • Minimum age requirement is 18 years.
  • Ability to communicate verbally and written English.
  • Sign language is an added asset.
  • Must be able to work independently and responsibly with confidential information
  • Must meet Metro Homes Inc. and State Regulations requirement for pre-employment.
  • High School Diploma
  • Valid driver’s license from state of residence to drive Metro Homes, Inc. vehicles
  • Current CPR/FIRST AID
  • TME Certification
  • Must be able to bend, stoop push, reach, lift, sit, walk for periods of time.
  • Must be able to utilize proper body mechanics while assisting with transfers of people weighing up to 250 pounds.
  • Individuals must have the ability to complete all training requirement and demonstrate competency with 80% on written/computerized exams.

Responsible for providing assistance to the people supported physically, socially and psychologically and completing required training documentation for compliance with State/Federal regulations or standards of service and general quality of service and safety of the people supported served in the programs as per the Policy and Procedure of the Agency and the governing agencies. These tasks are complex and required negotiation and communication skills, organizational abilities, advocacy skills and overall business sense. 

Key Functions:

  • Ensures implementation of nursing care plans by completing prescribed medical treatments
  • Ensures implementation of nursing care plans/healthcare plans by obtaining vital signs, weight and height of individuals every month and as needed
  • Ensures implementation of nursing/healthcare plans by setting up, dispensing and documenting administration of ordered medication and/or treatments
  • Ensures implantation of nursing/healthcare plans by monitoring side effects and effectiveness of medications/treatments
  • Providing total care of individual including going on vacation and community outings.
  • Maintaining strict aspiration precaution 24 hours.
  • Completes mediation administration record, PRN and charting requirements, monthly summary.
  • Works closely with nursing supervisory staff in reporting pertinent medial information.
  • Assists in ongoing in-service programs related to health services.
  • Transcribes and/or ensures physicians phone orders, reviews laboratory and other medically related reports, and consults prior to filing in the person supported records.
  • Ensures routine documentation of individual health status in nurse’s notes
  • Ensures provisions of needed adaptive equipment.
  • Dependable, accepts responsibility for attendance and punctuality.
  • Flexible, adjusts to workload to accommodate changes in priorities and guidelines.
  • Handles information in a confidential manner.
  • Cooperates in internal and external investigations and provide attachments of the EPD and incident management coordinator promptly when needed.
  • Attend all ISP meeting and plan of care meetings whenever scheduled.
  • Follow-up with all medical appointments and make sure that all medications are obtained from the pharmacy in a timely manner.
  • Review all MAR and POS monthly and make sure that orders are signed by the PCP before the 1st of each month.
  • Make sure all cycle medications are received from the pharmacy, checked and distributed before the cycle ends.
  • Provide G-tube feeding, and care as ordered.

Qualifications:

  • Experience in providing services and support to the people supported with developmental disabilities preferred.
  • Minimum age requirement is 21 years.
  • Ability to communicate verbally and written English. Sign language is an added asset.
  • Must be able to work independently and responsibly with confidential information.
  • Must meet all of Metro Homes, Inc. and State Regulations requirements for pre-employment.
  • Valid driver’s license from state of residence, valid/current First Aid and CPR, LPN license for DC.

General Summary:

Metro Homes, Inc. seeks a Registered Nurse Supervisor to access health problems and needs of person(s) served.  Develop and implement Health care management plans and maintain medical records.  Administer nursing care to person(s) served.  Advise person(s) served and staff on health maintenance disease prevention and provides case management.  Investigation of incidents after acquiring level one training for incidents. 

Key Functions:

  • Ensures the inclusion of health issues in each service plan which is based on thorough, comprehensive nursing/health assessments.
  • Ensures the implementation of nursing/health services and programs to maintain the health and wellbeing of individuals served.
  • Monitoring the provisions of quality nursing/health services to maintain the health and wellbeing of individuals served while ensuring expedient response to identified health needs.
  • Develops and maintains cooperative relationships and/or effective communication among staff, other health providers, state and public agencies.
  • Ensure the development and periodic review of health and nursing related policies and procedures that meet regulatory requirements and yet be responsible to individual needs and desires.
  • Ensures the timely and accurate recording of health issues, programs and services for individuals served by staff.
  • Conduct in-service and orientation training to staff.
  • Provides in-service in all areas of health services
  • Train staff on medication and side effects of medications.

Qualifications:

  • Bachelor’s Degree in Nursing required
  • Three (3) years of nursing experience with DDA/DDS population preferred.
  • Supervisory experience preferred.
  • Active RN License, CPR, First Aid and current health certificate
  • Evidence of effective professional writing, communication and clinical skills.
  • Computer literate.
  • Ability to utilize proper body mechanics while assisting with transfers of individuals weighing up to 280 pounds.
  • Ability to bend, stoop, push, pull, reach, sit, and walk for periods of time.
